Referral process
Referral forms can be downloaded from the main Agencies page, emailed on request or faxed on request.
Please fill in the form as accurately as possible we realise that agencies have many forms to fill in but it assists us to assess the young person better if you give full answers to all the questions.
Please return to the M3 office at the address below electronic copies are also suitable, but faxes do not always capture all the information so please send a hard copy if you do this. Remember, we cannot accept and process referral forms that have not been signed by the young person.
Young people will be invited for assessment as and when a possible match becomes available.
Unfortunately due to the large geographical area we are now covering, the Project is not always able to come out to young people for assessments. Please ensure the young person is supported to attend their assessment.
Once assessment has taken place, successful applicants will be matched to a host provider and offered an introductory meeting within a few days.
If a match is not available straight away, the young person can have their offer of a placement held over till a suitable placement becomes available. This, however, may take some time and agencies are urged to explore other options of accommodation for the young person.
Unsuccessful applicants and their referring agency will be notified by email, letter or telephone.
